§ 6.08.000. Form of application.

(a)

Every application for a license required by this title shall contain the following:

(1)

The name and mailing address of the applicant; if the applicant is a corporation, the name shall be exactly as set forth in its articles of incorporation; if the applicant is a partnership, the name and address of each general partner shall be stated; if one or more of the partners is a corporation, the provisions of this section as to a corporate applicant apply;

(2)

The names of all other persons having an interest in the enterprise;

(3)

If the applicant is a corporation, the name and mailing address of an officer who is duly authorized to accept the service of legal process;

(4)

The telephone number of the applicant;

(5)

The email address of the applicant;

(6)

The name under which the applicant will be doing business;

(7)

The nature of the business;

(8)

The exact location of the business or the route or territory to be covered if no fixed place of business;

(9)

The residence of the applicant or, if a corporation, the principal place of business;

(10)

The amount of the license fee; and

(11)

A statement that the applicant will not permit or allow violations to any law or ordinance on the premises for which the license is applied.

All applications shall be signed or electronically signed by the applicant prior to the license being issued.
